This form takes 5 – 10 minutes to complete. Once your application is submitted, you will receive an order confirmation via email.
If you opted in for our archived service when you initially applied for deed poll, you can submit this application to receive further copies of the deed poll.
Legal copies of a Deed Poll from our archives can only be issued to the individual(s) named on the original document. A child listed on a Deed Poll may request legal copies, provided they have reached the age of 16.
If you lost your original Deed Poll and did not opt in to archiving service at the time of your initial application, you can request a Replacement Deed Poll by emailing support@deedpolldepartment.co.uk.
Please note, cancellations requested after submission are subject to a £12.50 administrative fee.
